- A completed Notice of Injury Form or C-1 Form

All new, minor work-related injuries will be evaluated and assessed comprehensively.
Authorization is required.

What do I need for my employees to use SNOHC for Workplace Telemedicine?
To access SNOHC’s Telemedicine services, you will need a smartphone, tablet, or computer with a webcam and microphone.
How fast is the response time?
After submitting an Authorization for Telemedicine Services form, a link will be sent approximately 10-15 minutes later.
Who can use this service?
Any business, organization, or company that has an employee who sustained a minor work-related injury within the state of Nevada.
